Band 5 - Community Nurses

We are seeking motivated and dynamic, caring and passionate Band 5 Community Nurses to work within the Integrated Care teams In Essex to cover three main areas: 1. Basildon and Wickford; 2. Brentwood and Billericay; 3. Thurrock.

Main duties of the job

The post holder will be a Registered Nurse with experience of working in the NHS. Community experience is desirable but not essential. We welcome applications from student nurses approaching the end of their training! The post holder will be responsible for patients allocated to them and will assess, manage, plan and deliver care.

The post holder will support their peers and senior nursing staff and be an effective team member whilst working on their own initiative. They will supervise pre-registration students and band 4 nursing assistants/assistant practitioners.

As a member of our community multi-disciplinary team, you will work within a primary care network in a neighbourhood team alongside Team Lead, Community matrons, care coordinators, District Nurses, Specialist Nurses, Therapists and Health Care Assistant’s to meet the needs of a complex community delivering patient care supporting early discharge and avoiding admissions to the acute sector.

This includes:
  • End of life care,
  • Managing the deteriorating patient,
  • Complex and simple wound care including VAC therapy and compression therapy,
  • Venepuncture,
You will be required to be committed to collaborative working and actively supporting and liaising with other health and social professionals and agencies.

The role will require you to be computer literate as all patients’ records are now electronic.

Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement
This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,136 and a maximum payment of £1,915 per annum pro rata).
