Under the guidance and support of the Clinical Lead, the Nurse Assessor will assess individuals for NHS Continuing Healthcare, in line with the National Framework for NHS Continuing Healthcare and NHS Funded Nursing care, revised July 2022. The Nurse Assessor will promote best practice and provide professional advice to Nursing Homes and other providers, promoting good relationships and liaising with colleagues from other disciplines both internally and externally to the Integrated Care Board (ICB).
The post holder will be required to carry out Checklist assessments and FNC reviews and may also be required to carry out the NHS CHC assessments.
As part of the review process, the Nurse Assessor will review the eligibility for the NHS CHC and FNC, as well as the care plan, to ensure that all care needs are being met safely and effectively. You will monitor and influence the quality of care that is provided to patients; drawing on previous skills and knowledge to improve the patient experience of care. You will provide professional advice to patients, carers and multi-agency colleagues, including signposting to appropriate services and resources.
The Nurse Assessor will contribute to the safeguarding adults from abuse process (SAFA) by undertaking reviews of individuals who have been identified as at risk of harm; as part of this process you will have a role in the monitoring and reviewing of placements/packages of care.

In addition to the basic salary, this post attracts a High Cost Area Supplement of 5% of basic salary subject to a minimum and maximum payment per annum (pro rata where applicable) in line with Agenda for Change.
